JOB DESCRIPTION
TNCSA is a provider of Independent Support Coordination (ISC) services for the Department of Disability and Aging (DDA). The Independent Support Coordination Program assists individuals with intellectual and development delays to live as independently as possible within their communities through the provision of community-based services. The caseload for this position will be primarily working with clients of Katie Beckett program, which helps kids in Tennessee with disabilities and complex medical needs under the age of 18.
RESPONSIBILITIES
- Case management services for individuals with developmental and intellectual disabilities,
- Conduct meetings, writing individual support plans and submitting those plans to DDA in order to request services,
- Monthly monitoring of services and completion of vital paperwork.
MINIMUM QUALIFICATIONS
- Bachelor’s degree – Preferred field social work, psychology, sociology or related field.
- One to two years of work-related experience.
- ISC or DDA experience and/or certification a plus. Knowledge of Katie Beckett Waiver Program is also a plus.
- Excellent written and verbal communication skills.
- Must be very organized and detail orientated.
- A valid motor vehicle operator’s license is required. Must have reliable transportation and current automobile insurance.
- Must authorize the release of any investigative and criminal records obtained by the Federal Bureau of Investigation and the Tennessee Bureau of Investigation to the Tennessee Community Services Agency.

Work from home, UNLESS a face to face visit is needed; therefore, must reside in the State of TN.
Paid according to State of Tennessee mileage reimbursement guidelines for any travel incurred.
